Protection of Personal Information of Students and Faculty at the Campus Life and Health Support Center

The Act on the Protection of Personal Information came into effect on April 1, 2005. Our Center has always handled personal information with the utmost care. Going forward, we will use the personal information of our users only for the purposes stated below, and will continue to handle it with the greatest diligence.

All staff who handle health-related information — including physicians, counselors, public health nurses, and nurses — are fully aware of the importance of personal information and are committed to maintaining strict confidentiality, so as to prevent any leakage of personal information. If you have any concerns about how personal information is handled at our Center, please do not hesitate to let us know.

1. Purposes of Use of Personal Information at Our Center

①Health Examination Results

  • Provision of medical, health consultation, and other services at our Center
  • Issuance of health certificates, proof of health examination attendance, and other certificates
  • Referrals to other medical institutions and responses to inquiries from medical institutions (only with the individual’s consent)
  • Prevention of disease onset and health management (lifestyle-related diseases, overwork, etc.)
  • Work environment management and work management

Note: When examination data from other medical institutions (e.g., results of comprehensive medical checkups) are submitted, they are used as health examination data.

② Information Obtained through Medical Consultations, Health Counseling, Counseling, etc.

  • Provision of medical, health consultation, and other services at our Center
  • Referrals to other medical institutions and responses to inquiries from medical institutions
  • Contacting and explaining to family members
  • Case reviews to improve the quality of medical consultations, health counseling, counseling, etc.

Note: For contacts with other medical institutions and contacts with / explanations to family members, the individual’s consent will be confirmed each time.

③Information Obtained through Occupational Health and Safety Activities

  • Prevention of health hazards, work environment management, and work management

④ Information Required to Be Reported by Law

  • Reporting obligations under the Industrial Safety and Health Act, School Health and Safety Act, Act for Prevention of Tuberculosis, Act on Prevention of Infectious Diseases, and other laws

  • Basic data for maintaining and improving the operations of our Center
  • Statistical aggregation that does not identify individuals
  • Research to improve the quality of medical consultations, health counseling, counseling, etc.

In principle, when conducting “clinical research involving human subjects,” it is necessary to explain the research to participants in writing or verbally and obtain their direct consent. However, according to the “Ethical Guidelines for Medical Research Involving Human Subjects” established by the national government, for research that involves no invasion or intervention and uses only information from medical records, it is not required to obtain direct consent from participants; however, it is required to disclose the research objectives and content and to ensure that participants have the opportunity to opt out. This is called “opt-out.”

In accordance with this policy, you can find the list of “clinical research involving human subjects” currently conducted by the Campus Life and Health Support Center from the research list below. The research disclosure section provides detailed information on each research project, along with the contact information of the principal investigator.

When conducting each research project, measures are taken to anonymize data so that individuals cannot be identified. However, if you or your proxy do not wish to have your medical information used in research, or if you would like to know more about the research content, please contact the principal investigator of each study.
